Testimonial Release
Purpose
Troopers may occasionally document its programs, events, community activities, and resident experiences through photography, video, audio, or written reflections.
These materials are used to highlight the mission of Troopers and raise awareness about the work we do in support of individuals experiencing homelessness, substance use disorder, and mental health challenges.
Documenting and sharing these moments helps us educate the public, advocate for change, and raise support for our mission.
Authorization for Media Use
By visiting and reading this notice, you authorize Troopers to take photographs, video recordings, and/or written testimonials that may include your image, likeness, name, voice, or story.
These materials may be used for educational, outreach, promotional, or fundraising purposes in a variety of formats, including but not limited to printed brochures, training tools, presentations, reports, newsletters, websites, social media platforms, and media outreach campaigns.
Organizational Representation
You also authorize Troopers to use and publish these materials in ways that help enhance the public image, visibility, and reputation of the Troopers organization and its leadership, including German Urrego, who serves as a representative and advocate for the program and its mission.
Any use of these materials will be professional, respectful, and aligned with our core values of dignity, equity, and recovery.
Voluntary Participation
Participation in media documentation is completely voluntary and is not a requirement for receiving services.
If you prefer not to be photographed, filmed, recorded, or featured in any way, you may opt out at any time by informing staff.
We will document your decision in your file and take every reasonable precaution to ensure that you are not included in any photographs, video recordings, or shared written materials.
Right to Withdraw Consent
If you choose to give your consent now but change your mind later, you have the right to withdraw your consent at any time by submitting a written request.
Once your request is received, Troopers will make every effort to stop using any future materials involving you.
Please note that we may not be able to remove previously printed or publicly distributed materials, but we will discontinue any future usage and publication.
Confidentiality and Privacy
This release does not give Troopers permission to disclose sensitive personal, medical, or protected health information about you.
Any media involving your participation will be created and shared in a way that respects your confidentiality, unless you explicitly agree to share personal aspects of your story.
Clarification and Questions
Participation in photos, videos, or testimonials is voluntary and not required for housing or services.
If you have any questions about this release or how your image or story may be used, please speak with a Troopers staff member before participating.
Acknowledgment and Consent
By visiting and reading this notice, you acknowledge and agree to the terms outlined in this Media & Testimonial Release Form.
You understand that participation is voluntary, that you may withdraw consent at any time, and that Troopers will use any materials professionally and respectfully in accordance with its mission and values.
